Agreement between User and LipoLoungeJAX.com

Welcome to LipoLoungeJAX.com. The LipoLoungeJAX.com website (the “Site”) is composed of various web pages operated by Sarah Boyd, Inc  (“Lipo Lounge JAX”). LipoLoungeJAX.com is offered to you conditioned on your acceptance without modification of the terms, conditions, and notices contained herein (the “Terms”). Your use of LipoLoungeJAX.com constitutes your agreement to all such Terms. Please read these terms carefully, and keep a copy of them for your reference.

LipoLoungeJAX.com is an E-Commerce Site and a brick and mortar business providing services and products.

Lipo Lounge JAX is a business specializing in Body Contouring, Massage Therapy and Health & Life Coaching. Services include many modalities of body contouring, massage therapy, physical assessments, and health, wellness, and lifestyle coaching as well as various related products.

Refunds

Cash Refunds will NOT be issued for:

• Services already rendered
• Services cancelled with less than 24 hours notice
• Gift Certificates
• Pre-paid Packages or Pre-paid Subscriptions
• Opened Products

If offered, Refunds will be issued in the form of Gift Certificates unless otherwise agreed upon in writing at the point of sale.

Cancellations

Lipo Lounge JAX requires a 24 Hour Notice of cancellation via Phone Call or Email. Failure to cancel with 24 hours notice will result in a Cancellation Fee of 50% of scheduled service. Failure to properly cancel an appointment prior to appointment start time is considered a No Call/No Show and will result in a Cancellation Fee equal to 100% of the total amount of the scheduled appointment or the forfeiture of a session from an equivalent active package, subscription, or gift certificate.

Electronic Communications

Visiting LipoLoungeJAX.com or sending emails to Lipo Lounge JAX constitutes electronic communications. You consent to receive electronic communications and you agree that all agreements, notices, disclosures and other communications that we provide to you electronically, via email and on the Site, satisfy any legal requirement that such communications be in writing.

Children Under Thirteen

Lipo Lounge JAX does not knowingly collect, either online or offline, personal information from persons under the age of thirteen. If you are under 18, you may use LipoLoungeJAX.com only with permission of a parent or guardian.
